Critical Corrected During Inspection PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating. Incorrect temperatures were stated for when re-heating chilli for hot holding stated temperature was less than 165F. Information provided by sanitarian at time of inspection.

Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Employee working in kitchen did not have signed illness agreement on file. PIC had signed at time of inspection.

Critical Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. Hand sink in front bar area, and in back bar area had items in them at time of inspection. PIC had items removed and sinks washed, rinsed and sanitized.

Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Baskets and scoops used in kitchen deep fryer had visible dried on food and residue. Baskets stored on top of tray with old oil and food debris. PIC pulled items and put into warewashing.

Corrected During Inspection Cooking and/or baking equipment not cleaned when required. Rack in toaster oven in kitchen by bar had visible burnt on residue. PIC had cleaned at time of inspection.

Critical Corrected During Inspection Food equipment surfaces not cleaned at required frequency. Holding socket for soda dispenser nozzle on front bar soda dispenser had visible residue. PIC washed rinsed and sanitized.

Repeat Corrected During Inspection Utensils and equipment contacting non-TCS foods not cleaned at required frequency. Ice scoop for bulk ice machine and tray stored on are not cleaned once every 24 hours. PIC had item washed, rinsed and sanitized.

Critical Corrected During Inspection Food thermometers are not accurate. Two thermometers in main kitchen observed to have a ten degree difference in reading of ambient air. PIC removed from service till they are calibrated.

Conducted and reviewed inspection with Kitchen Trustee Zondra Adkins.
Discussed sanitixer test strips need to not be expired.
Discussed tile on floor in kitchen are beginning to deteriorate and should be replaced when no longer smooth, durable and easily cleanable.
Discussed that if south bend ovens are not going to be repaired, the need to be removed.
Discussed need for ODH manager certificate. Reciprocity form provided at time of inspection.
Level IV activity, facility cools and re-heats in bulk more than once a week.
