Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. At time of inspeciton, the salads on a tray cart were observed uncovered inside the walk in Kolpak and subject to sneeze, splash, or dust. CORRECTED, salads covered with wax paper
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. At time of inspeciton, the Shelly Steel chillers on the food line had not thermometer CORRECTED, thermometers brought in for chillers
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.
3717-1-04.4(B) / Cutting surfaces.
Corrected During Inspection The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. At time of inspection, red cutting board had excessive scarring and needed replaced. COIRRECTED, red cutting board disposed of
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. At time of inspeciton, center serve line Smith dairy cooler had excessive ice build up that needed defrosted and cleaned
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.4(A) / Repairing.
Repeat The physical facilities are not being maintained in good repair. At time of inspeciton, walk in Kolpak freezer had leak detected and needed repair

Conducted inspection with PIC Audrey Keiffer; reviewed inspection findings with PIC According to PIC, romaine lettuce has been replaced with shredded lettuce as a result of the call back Discussed cross contamination with PIC and reviewed employee reporting agreement. PIC stated that food service does not reheat bulk quantities of food more than two times a week.
