No air gap or approved backflow prevention device on the plumbing system. Observed hose on mop sink under constant pressure without proper backflow prevention.
3717-1-05.1(D)/Backflow prevention - air gap.
Insufficient air gap between the flood rim and the water supply inlet. Observed the spray nozzle hanging below the flood rim level of the 3 compartment sink.

Discussed and conducted re-inspection with Carrie, Manager. Manager stated the red hose in the mop sink will be turned off in between uses. Inspection report sent via email.
