Equipment components are not intact, tight or properly adjusted. Observed gasket on door of walk in freezer to be broken and in disrepair. Repair or replace gasket. -Correct by next standard inspection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed plastic sliders in beverage retail coolers to contain visible residue. -Clean by 3-18-24.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed ice build up on walk in freezer door. Remove ice build up and thoroughly clean. -Correct by 3/18/24.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed finish on handle of walk in beer cooler to be peeling and to contain rust. Reseal or replace to ensure surface is easily cleanable. -Correct by next standard inspection.

Corrected During Inspection RFE constructed, altered, and/or equipment added without prior approval from the licensor. Observed Vollrath hot holding merchandiser added to self service counter. At time of inspection, observed Vollrath to be commercial grade (ETL sanitation listed) and approved for use in facility. Ensure new equipment is sent to specialist at Richland Public Health for review and approval.

Comments
Inspection conducted with manager/PIC. Risk level II activities include holding for sale potentially hazardous foods at the same at the same proper holding temperature at which it is received. At time of inspection, PIC states facility is now hot holding pre-cooked breakfast sandwiches as a grab in go option a the self service retail counter. PIC states facility receives pre cooking sandwiches frozen, thaws in walk in cooler, reheats in microwave to an internal temperature of 165*F and hot holds in merchandiser at 140*F. Discussed that hot holding is a risk level 3 activity and facility will move to a risk level 3 food service license. Inspection report emailed.
