Critical Corrected During Inspection Improper use of time as a public health control (4 hours). Observed cheese on make line with time stamp of 7 am and was past 4 hour threshold, employee discarded cheese. Observed time stamp label on cut lettuce to not have time marking. Employee verbalized time lettuce was placed on make line and appropriately labeled during inspection.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Corrected During Inspection Food employee(s) not wearing a hair restraint. Observed food employees with facial hair not wearing beard nets and food employee to have loose hair hanging from under hat. PIC had food employee don facial hair restraint and secure loose hair under hat at time of inspection.

Fixed equipment not properly sealed or spaced for cleaning. Observed seal along floor and wall junction of mop sink to be in disrepair. Reseal by next standard inspection.

Non-food contact surfaces of equipment are unclean. Observed back of clear plastic ice dispenser on beverage machine in dining area to have visible build up. Clean by next standard inspection.

Facility not maintained clean. Observed wall behind prep sink and seal along wall junction of prep sink to contain visible black build up. Clean by next standard inspection.

3717-1-06.4(A) / Repairing.
Corrected During Inspection Physical facilities not maintained in good repair. Observed ceiling tile missing in back storage area. PIC states it just got slid to the side and replaced at time of inspection.

Complaint on 06-Oct-2022 :
Inspection conducted with Tim, Manager/PIC. Food employees observed without facial hair restraints and without effective hair restraints at time of inspection. Reviewed code section 2.3(C) regarding use of hair restraints for food employees during inspection. Manager states he will order beard nets and ensure employees with facial hair are wearing them. Manager did provide food employee with bandana to wear as facial hair restraint as corrective measures at time of inspection. Discussed ensuring TCS foods on make line are properly marked with either time food is pulled from temperature control (41*F or less) or for time of discard/ Also reviewed importance of these food being discarded once time is finished versus placing back in cooler. Inspection report emailed.
