Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. PIC unable to locate signed copies of employee illness reporting agreements at the time of inspection. Provided RPH's employee illness reporting agreement form during inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed residue inside the nozzle of the juice wand. PIC had juice wand cleaned at the time of inspection.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Repeat Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed a bag of turkey lunch meat with a slice date of 10/8 in the make table cooler. PIC voluntarily discarded turkey at the time of inspection.

3717-1-04.1(L) / Ambient air and water temperature measuring devices - accuracy.
Ambient air and water thermometers are not accurate. Thermometer inside of the walk-in cooler read 24*F at the time of inspection. Actual ambient air temperature inside of the walk-in cooler was 36-38*F at the time of inspection. PIC stated a new thermometer will be ordered. Correct by next inspection.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Corrected During Inspection [52] Improper storage of single-service and single-use articles. Observed a box of paper cups stored on the floor inside of the storage closet. PIC placed a plastic crate under the box of cups at the time of inspection.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No quat sanitizer test kit available. Correct by next inspection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed a build-up of ice inside of the ice cream freezer. Correct by next inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed residue on the wall behind the juice & soda wand dispenser. Correct by next inspection.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Light intensity less than fifty foot candles in required areas. Lighting next to the fryers at 10 foot candles due to burnt out light bulb. Correct by next inspection.

Discussed inspection with dietary director Patty. Left an extra copy of ODH's procedures for clean-up of vomit/fecal incidents. Emailed inspection report.
