Equipment components are not intact, tight or properly adjusted. -Observbed the kick plate on the cooler containing milk and eggs to be broken/detached.

-Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Correct By: 27-Feb-2018
3717-1-06.4(F) / Drying mops.
Observed mops being dried using an unapproved method. -Observed mop left in mop bucket, unable to air-dry properly.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Correct By: 27-Feb-2018

PIC and GM unable to determine whether Employee Illness Reporting Agreements exist at facility.
-Provided RPH document.
-If no existing agreements can be located, make copies and have each employee sign one.
-Keep signed copies at store for future inspections.

PIC and GM unable to determine whether Vomit Response Procedure exists for facility.
-Provided RPH document.
-Keep this page and all supplies listed under step #2 at the facility at all times.

Noted some presence of water in basement/old rest room area. Water depth up to 1" in some places may be due to a spilled mop bucket or possibly flooding.
-Identify and address this issue by time of next inspection.
