Critical License holder did not require employee(s) to report symptoms, an illness diagnosed by a health care provider, or high risk situations as specified by law. Operator did not have a verifiable employee illness reporting policy and PIC stated that she did not know the reportable symptoms or illnesses that would constitute an effective employee illness reporting policy.
To prevent the transmission of foodborne illnesses, the license holder shall require food employees and conditional employees to report to the person in charge information about their health as it relates to diseases that are transmissible through food.

Critical The person in charge was unable to demonstrate proper knowledge of food safety and prevention including holding temperatures, sanitizer concentration testing, and employee illness reporting.
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection.

Corrected During Inspection In-use utensils are improperly stored. Observed ice scoops in 2 machines stored with handles in the ice.
During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored as described in this rule.

Corrected During Inspection Observed improper storage of food items. Observed candy stored in a cardboard box on floor with accumulated debris.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.

Corrected During Inspection There is no test kit available for measuring the concentration of the sanitizer. Observed facility to have chlorine test strips but to be using quaternary ammonium as its warewashing sanitizer. Reviewed sanitizer testing procedure and gave PIC 5 quaternary ammonium test strips.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.

Corrected During Inspection Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s) in kitchen.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.

Observed a build-up of dirt, debris and dead pests in dry storage room on floor.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
