Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Observed no employee to have level two certification in food protection at time of inspection.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.

3717-1-04.1(Y)
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. Observed no thermometer present in deli cooler at time of inpsection.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.

Observed expired test kit that can no longer accurately measure the concentration of the sanitizer.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.

Non-food contact surfaces of equipment are dirty. Observed non-food contact surface of pop machine ice dispenser and nozzles to have an accumulation of debris.
Nonfood-contact surfaces of equipment shall be kept clean.

Critical Observed equipment food-contact surface of deli slicer had an accumulation of food debris at time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
