Observed food items stored in bakery walk in cooler stored on the flour in original packaging.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
Observed donuts displayed as grab and go without labeling or menu with label identifying possible allergens.
All grab and go food shall have the following listed on the label: common name, ingredients, weight or volume, allergens, and the packer, distributor or manufacturer. Please place a menu with all aspects described above. Place a common name on donuts displayed to identify what label references what donut in question.
Observed an accumulation of frozen condensate in rear left corner of walk in ice cream freezer.
The physical facilities shall be maintained in good repair.
