Corrected During Inspection In-use utensils are improperly stored, observed scoop with handle touching food.
During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored as described in this rule.
3717-1-04.4(A)(1)
Equipment and/or components are not maintained in good working order, observed damaged wiring in walk in freezer.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Non-food contact surfaces of equipment are dirty, observed food accumulation in reach in freezer on prep line.
Nonfood-contact surfaces of equipment shall be kept clean.

Corrected During Inspection Utensils and/or equipment were not properly rinsed, observed employee not rinsing dishes. .
Washed utensils and equipment shall be rinsed so that abrasives and cleaning chemicals are removed or diluted through the use of water or a detergent-sanitizer solution and by using one of the procedures specified in this code. Employee was educated on proper procedures and dishes were re-washing properly.

The floor and wall junctures are not properly coved or closed, or floor drains were not provided, observed missing coved molding in walk in cooler.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
