Facility does not have an employee with manager certification in food protection.
Pic was not familiar with the name of the manager certificate holder. Pic did not know if that person still worked for the business. Facility must have an employee with manager certification who is available to train staff for food safety.

Corrected During Inspection Critical Repeat Food employee(s) not washing hands in approved handwashing. Employees are washing hands in 3 compartment sink. Hand sink is not connected to plumbing at this time.
Pic removed items from sink located beside hand wash sink. This sink will now be utilized as the hand washing sink.

Corrected During Inspection Critical Repeat Handwashing sink not accessible. Observed hand wash sink not connected to plumbing. Employees are washing hands in the 3 compartment sink. Handwashing may only occur in a handwashing sink. Pic is now using sink located beside previous hand washing sink.

Critical Equipment food-contact surfaces or utensils are unclean. Observed excessive accumulation of debris on food contact surface of ice machine. To prevent contamination, food contact surfaces shall be clean to sight and touch.

Cold holding equipment thermometer was missing in reach in cooler.

Equipment not approved by a recognized testing agency. Observed crockpots being used to hot hold burgers, hot dogs and sauce. Observed proper hold holding temperatures at time of inspection.

Walls not smooth and easily cleanable. Observed exposed wood by sinks in kitchen. Surfaces exposed to moisture shall be smooth, easily cleanable and nonabsorbent.

Repeat Physical facilities not maintained in good repair. Observed damaged and missing floor tile in food prep area, 3 compartment sink and handwash sink.

Repeat Light intensity less than fifty foot candles in required areas. Light meter reading of 2 to 12 FC in food prep area and dish washing area.

Current FSO license not displayed. Observed previous years license displayed at time of inspection.

FSO did not have a person in charge per shift with the person in charge certification in food protection at time of inspection.

Comments
Discussed maintenance of facility including repairs of flooring, covering exposed walls, and plumbing. Person in charge stated that the kitchen is still scheduled to be remodeled. Discussed plan review process with pic. Plan review provided at time of inspection. Reviewed manager and person in charge food safety requirements. Plumbing changes require a plumbing permit and must be completed by a registered plumber. Repeat violations may result in an Administrative Hearing.
