Observed a FSO that did not display their license at the facility.
A licensee shall display the license for that food service operation at all times at the licensed location; Each operator of a mobile FSO shall conspicuously display the name of the operation, city of origin, and area code and telephone number on the exterior of the mobile unit with the individual lettering measuring at least three inches high by one inch wide.

Observed nonfood-contact surfaces constructed of unapproved materials, observed cardboard being used for shelving on equipment/food storage rack.
Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be smooth and durable.

There is no test kit available for measuring the concentration of the sanitizer.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.

Corrected During Inspection Observed an outer openingdoor without proper protection against the entry of insects and rodents, observed oustide door to main facility open, and door on outside storage to be open.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
