To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(B)
Critical Food equipment surfaces are not cleaned at the required frequency, observed accumulation of debris on food contact surface of ice machine.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule.

Discussed cleaning frequencies with PIC. PIC took the level two training on 12/11/17 and is waiting on ODH certificate. Sent employee illness and clean-up procedures to PIC by email.
