Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Non-food contact surfaces of equipment are dirty. Observed accumulation of dirt and debris on handles of reach-in coolers and freezers.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-04.8(E)
Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. Observed single use articles (Pizza boxes) stored on the carpeted floor of the dry storage area.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.

Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Observed an exterior door near storage area to have a gap at the bottom that could allow pests to enter the building. I do not observe any pests in the building.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.

Observed a build-up of dirt and debris throughout facility on walls and floors.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
