Corrected During Inspection Critical Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed raw eggs stacked above ready to eat foods. To prevent contamination, please store raw eggs below ready to eat foods.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-04.4(A)(1)
Corrected During Inspection Equipment and/or components are not maintained in good working order. Observed prep cooler to be malfuctioning at time of inspection and not keeping temperature below 41 degrees F. PICs were in the process of discarding food from this cooler and taking it out of operation until it could be serviced.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Critical Repeat Equipment food-contact surfaces or utensils are dirty. Observed ice machine to have an accumulation of residue and debris on interior food contact surface at time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
Comments
Reviewed proper hot and cold holding, heating, and reheating temperatures. Also reviewed employee health policy, backflow prevention in mop sink, and proper handwashing procedures including when to wash and how to wash. Both PICs demonstrated good knowledge on these subjects at time of inspection. Upon arrival at facility, PICs were in the process of discarding multiple food items that were out of temperature due to a malfunctioning prep table cooler. This demonstrated good knowledge of proper cold holding procedures and the importance of discarding food when it has not been held at proper temperature due to equipment malfuction. Thank you.
