Corrected During Inspection Food storage containers are not properly labeled, observed food storage containers without proper label.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.5(A)(3)
Non-food contact surfaces of equipment are dirty, observed push cart with accumulation of debris.
Nonfood-contact surfaces of equipment shall be kept clean.

Observed no covered receptacle in women's restroom, observed that womens restroom did not have a covered receptacle.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

Observed waste receptacle drains that did not have plugs, observed that dumpster did not have a drain plug in place.
Drains in receptacles and waste handling units for refuse, recyclables, or returnables shall have drain plugs in place.

Observed a build-up of dirt and debris, observed accumulation of debris on walls in dishwash area.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
Comments
Inspection conducted due to a complaint. Complaint stated that customer ordered a taco salad around 2 PM on 1/9/2018. Customer bit into a hair clip. PIC stated that they have no female kitchen employees and that all male kitchen workers have short hair. All kitchen employees were wearing proper hair restraints at time of inspection. Hair clips of any kind were not observed at time of inspection.
