Critical Observed employees are not informed in a verifiable manner of their responsibility to report information about their health. Corrective Action: I gave PIC a copy of the RCHD Employee illness policy to be signed/dated by all Food service employees after reviewing.

Corrected During Inspection Observed paper in the prep unit that when closed would be in direct contact with the food. Corrective Action: PIC corrected this by adhering the paper to the unit.

Observed facility's sanitizer test strips to be out of date.
