Equipment and/or components are not maintained in good working order. Observed multiple pans to be dented in corners. Also observed a walk-in cooler door to be damage with exposed insulation on edge of the door.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Non-food contact surfaces of equipment are dirty. Observed the interior nonfood-contact surface of an ice machine to be dirty. Please ensure that nonfood-contact surfaces of ice machine are cleaned at a frequency recommended by the manufacturer, or as often as needed to prevent an accumulation of debris, whichever is more frequent.
Nonfood-contact surfaces of equipment shall be kept clean.

Facility was observed to be clean at time of inspection. Person in Charge (PIC) demonstrated a wide breadth and depth of knowledge related to food safety, including proper cooking, holding, and reheating temperatures, cooling procedures, date marking, disposition of food, handwashing procedure, cleaning and maintenance of facility. Food was not being cooked or served at time of inspection.
