[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
The licensee /authorized representative was not keeping or maintaining operational records.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met. Observed flow rate at 40 gpm at time of inspection. PIC back-washed sand filter, and increased flow rate to 70. Due to the size of the pool, the minimum required flow rate is 114.5.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(q) / Depth markers/deck warning signs are maintained
Depth markers/deck warning signs were not maintained in a clearly legible/visible manner.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated. Discussed repainting faded depth markers at time of inspection with PIC.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Observed non-self closing door to pool at time of inspection.
All perimeter barriers shall be with gates or doors that are self-closing and lockable unless otherwise premitted by law.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8. Observed pH at 8.0 at time of inspection.
Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.
Ensure that a sign informing patrons of the location of the emergency device is always visible in the pool area.

Comments
Operator noted that alterations were made to pool with Ohio Department of Health (ODH) approval. Operator noted that a hired contractor made alterations and ODH noted that proposed alterations were not considered significant and an Equipment Replacement Notification was not necessary to file. Previous inspections note presence of no main drain, however two main drains were observed in deep end of pool at time of inspection. TCHD staff will be reaching out to ODH for confirmation of alterations.
