Repeat Observed there was standing water on the floor areas of the equipment/ filter room at time of inspection.
Remove the standing water and modify the floor so that water will flow to the drain and not be able to pool on the floor.

Repeat Observed the gates were not self-closing and lockable at time of inspection.
Repair gates or doors in order to be self-closing and lockable.
3701-31-04 (B)(6)(u) / Ventilation system is maintained/ free of hazards
Repeat Observed the Components of the ventilation system was not working in the pump room at time of inspection.
Repair/replace components of the ventilation system to good working condition and without hazards.
