Observed ladder that was not secure in deep end of pool at time of inspection.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Observed deck surface on south west corner of pool cracked and uneven at time of inspection.
Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.

A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the pool at time of inspection.
Provide a proper floatation device at all times.
