[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Observed public swimming pool and/or spa was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH) at time of inspection: this includes, but is not limited to, new filtration and for the spa that requires equipment replacement notification (ERN) documentation.
Contact ODH to obtain approval for any construction/alterations before operating pool.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Observed the licensee /authorized representative was unable to provide current operational records upon request.
All facility records must be maintained and available for review upon request.
3701-31-04 (D)(7) / Public spa shall have a functional automatic chemical controller
Critical Observed an automatic chemical controller (ACC) unit that was available, but not up and operating at time of inspection. It is required to have an ACC unit in constant operation for a spa.

3701-31-04 (E)(2)(b)(ii) / A back/spine board with straps and head immobilizers
Observed a back/spine board with straps but missing the required head immobilizers at the guarded pool and/or spa. Ensure that the board has a head immobilizer attached at all times.
Provide a back/spine board with straps and a head immobilizer at the pool at all times.
