Secondary means of entrapment/evisceration prevention was not provided/observed. There are seceral methods to prevent entrapment for apublic pool. Provide documentation regarding the presence of current entrapment prevention mechanisms or contact the Ohio Department of Health for plan review prior to installation of the entrapment/evisceration prevntion unit. It is also recommended that the public swimming pool not be in operation until the violation is corrected and/or documentation is provided regarding exisitng equipment or design criteria that qualify as a means of preventing entrapment/evisceration.
Submit plans to the Ohio Department of Health for review and approval for the secondary means that will be employed on the pool.

Deck surfaces had standing water or other hazards. Observed damage pool decking repair. Repair the potential safety hazard to prevet accidents.
Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
At the time of inspection, the self latching mehaism was not functional. Repair/replace the self latching mechanism.

Repeat Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
