3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Facility changed disinfectant from calcium hypochlorite to sodium hypochlorite. Correction: Submit a public pool equipment replacement notification report to ODH for disinfection change.

3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Observed 6-9 inch gap in the perimeter barrier at the double gate near pavilion.

3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical Corrected During Inspection Doors of equipment/chemical storage area were open to allow for ventilation of the area at the time of inspection.
Ensure doors remain closed and locked when authorized personnel are not present. (PIC explained the doors are not commonly left open, a net was provided at the time of inspection and a lifeguard was to be posted at the area to prevent unauthorized entry for the time being).
