3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
The facilities were not maintained in a safe, clean & sanitary, and in good repair. Observed perimeter drain guard cracked at the time of inspection. Ensure pool facility is in good repair.

3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Combined chlorine was not tested daily at pool side prior to use. Observed combined chlorine only tested once a day and not every 4 hours while the pool open for use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Repeat pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features. Observed pH only recorded once daily and not every 4 hours. Pool opens from 6am to 12pm.
pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use pH must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the pH must be tested at 1 special feature every 6 hours the sprayground or spray feature is in use.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met. Observe flowmeter not working therfore the turnover rate could not be determined at the time of inspection.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(k) / Outlet drain covers are secure/meet ANSI/ASME A112.19.8-2007 requirements.
Critical Observed outlet drain covers not secure/meet ANSI/ASME A112.198-2007 at the time of inspection. Ensure outlet drain is covered and secure to prevent environmental contamination. In addtion make sure facility has certification for Virginia Graeme Baker(VGB) to ensure compliance

3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Observed entry/exit door between food facility and pool was not locked at the time of inspection. Ensure all gates and doors are self closing and lockable to prevent unauthorized bathers.

Test kit reagents were not fresh or stored properly. Observed test kit reagents not fresh at the time of inspection.

3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Observed line was less than 30 foot at the time of insprction. Ensure line at least 30-60 foot.
Provide a proper floatation device at all times.
