Facility changed filter media from sand to glass. Correction: Submit a public pool equipment replacement notification form to ODH for the change of filter media.

3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Gate on perimeter barrier is not self-latching. Correction: All gates or doors on the perimeter barrier must be self-closing and self-latching.

3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Repeat Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.
Ensure that a sign informing patrons of the location of the emergency device is always visible in the pool area.

Comments
As per verification with ODH pool division engineering department, the existing 24" x 24" drain cover on the pool is considered in compliance, and no additional requirements for SVRS are needed at this time.

Ensure Type IV personal flotation device with 30-60 ft of one-quarter inch line is readily visible and easily accessible on the pool deck.
