[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
At time of inspection, observed public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). This includes, but is not limited to, the Pool filter and pump (both had been replaced with a different make and model).
Contact ODH to obtain approval for any construction/alterations before operating pool.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Observed the licensee /authorized representative was not able to provide current operational records for review at time of inspection.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(6)(i) / A means of determining rate-of-flow on all water pumps
Observed a means of determining rate-of-flow was not found for the pool at time of inspection.
Properly install a flow meter or have a pump curve available for every water pump used for the pool.
3701-31-04 (E)(2)(b)(ii) / A back/spine board with straps and head immobilizers
A back/spine board with straps did not have a head immobilizer at time of inspection.
Provide a back/spine board with straps and a head immobilizer at the pool at all times.

-At time of inspection, the ACC unit was not hooked up for operation of either the pool or the spa.
