[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Repeat Facility changed disinfectant from calcium hypochlorite to sodium hypochlorite. Correction: Submit a public pool equipment replacement notification form to ODH for disinfection change.

3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
The facilities were not maintained in a safe, clean & sanitary, and in good repair. Observed leaking plumbing fixtures in the family restroom and leaking shower faucet in the women's restroom.

3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Water temperatures were not being recorded daily.

3701-31-04 (B)(4)(a)(vi) / Total alkalinity is tested weekly
Total alkalinity was not tested weekly.

3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Observed 6-9 inch gap in the perimeter barrier at the double gate near pavilion.

3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical Repeat Corrected During Inspection Doors of equipment/chemical storage area were open to allow for ventilation of the area at the time of inspection. Correction: PIC closed and locked the equipment/chemical storage doors at the time of inspection.

3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Total alkalinity was less than 60 ppm.
Raise the total alkalinity to at least 60 ppm.
3701-31-04 (E)(3)(g) / Spray ground warning sign is posted.
Spray ground warning sign with the required language was not posted.
Post the spray ground warning sign at the spray ground during all times of the year the spray ground is available for use. Post the following warning sign using the same or similar language -DO NOT USE POOL WHEN YOU HAVE DIARRHEA -WATER IS NOT MEANT FOR DRINKING -WASH HANDS AFTER USING THE REST ROOM OR CHANGING DIAPERS -TAKE REGULAR REST ROOM BREAKS, CHANGE DIAPERS ONLY IN A REST ROOM
3701-31-04 (E)(f) / Slide safety recommendation sign is posted and visible
The safety recommendation sign for the slide(s) was not posted and/or not visible.
Post the safety recommendation sign for the slide(s) in a prominent location at all times the pool is available for use. The postings shall contain the following information using the same or similar language unless otherwise specified by the manufacturer: -Single user (1)Water slides. Only one user at a time is permitted: forming "chains" is prohibited (2)Recreation slides. Only one user shall be on the platform at any time -Always check to see if the landing area is clear before entering the slide. Users shall follow directions from the dispatcher -Users must exit the landing area of the slide immediately -Users shall ride feet first at all times; stopping or changing position in prohibited -The minimum user height shall be posted. This measurement shall be the manufacturer's recommendation or six inches above the static water level of the pool based on the depth of the public swimming pool immediately beneath th eflume on the slide -For speed slides, users shall remain flat -Life jackets or other flotation devices are prohibited other than those designed for the slide and used as directed by the manufacturer
