3701-31-04 (B)(1)(e) / Automatic Chemical Controller (ACC) shall function properly.
Critical Repeat Automatic Chemical Controller (ACC) was not functioning properly at the time of the inspection. ACC not functioning at the time of inspection.
Do not operate the pool when the ACC unit is not functioning properly. Ensure that the ACC unit is functioning properly prior to placing the pool back into operation.
3701-31-04 (B)(6)(a) / Pool structure is water tight
Repeat The spa structure was cracked, in disrepair and/or leaking water.
Repair the spa structure to a water-tight system. If necessary, consult with the Ohio Department of Health regarding the repair.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Repeat pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (D)(7) / Public spa shall have a functional automatic chemical controller
Critical Repeat Auto Chemical Control not functioning at the time of inspection.
Ensure spa shall have a functional automatic chemical controller
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Repeat Emergency device (telephone) was not working in the pool area.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Repeat Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.
Ensure that a sign informing patrons of the location of the emergency device is always visible in the pool area.
3701-31-04(D)(6)(a)(5) / Spa bromine disinfectant residual is above 4 ppm.
Critical Repeat Spa bromine disinfectant residual was below 4 ppm.
Do not operate the spa until the bromine disinfectant residual is at 4 ppm or above.

Comments
Efia and Micayla did a follow inspection for the spa and the Bromine was in compliance above 4, reading 15 at the time of reinspection. Reiterated to facility to make sure paper work was sent to TCHD regards to ACC being ordered. Tuscarawas County Health Department still waiting on a response from facility, pending follow up.
