| 3701-31-04 (B)(4)(a)(ii) Combined chlorine was not tested daily at pool side prior to use. Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use, combined chlorine must be tested every 12 hours while the pool is in use.  |
| 3701-31-04 (C)(6) Pool water temperature was above 90 degrees Fahrenheit without approval from the Director of Health. Either reduce the water temperature of the pool to below 90 degress Fahrenheit or seek approval to operate at a higher temperature from the Director of Health. |
| 3701-31-04 (D)(8) Comparator tube was observed to have excessive buildup and residue. Test Kit was also missing the R-0003 reagent to test for combined chlorine. Thoroughly clean or replace comparator tube. Order the R-0003 reagent to be able to test for combined chlorine daily. |
