| 3701-31-04 (B)(6)(p) Deck surfaces had standing water or other hazards. Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval. Missing concrete near East end of pool in 12 ft depth section in between ladders. Replace to prevent accidental tripping into pool area. |
| 3701-31-04 (B)(6)(t) Critical Door to chemical storage area and equipment room was left open and unattended with patrons on site. Even during lifeguard breaks an authorized person should be responsible for ensuring no patrons accidentally wander into chemical storage areas. |
